All matriculated students taking six or more credits are required to have a completed health form including an up-to-date immunization record on file in the Wellness Center.
When circumstances arise so that a student’s physical or mental health condition requires services beyond the normal and reasonable scope of the University's capabilities, or when a student’s health status is deemed by the Dean of Students to present a serious threat to the welfare of the student and/or members of the University community, the student may be required to temporarily leave the University pending resolution of his or her condition. See related Mandatory Leave policy. The Dean of Students shall act on the basis of reliable information obtained from appropriate professional sources.
All medical records are maintained in the Wellness Center and no one except the Wellness Center staff has access to records without the student's authorization.
