Identity/Statement of Educational Purpose Instructions
Part of the verification process may require that we verify your identity and that you sign an Identity and Statement of Educational Purpose. You can satisfy this requirement in one of two ways.
Appear in person at Castleton University Financial & Registration Services and present a valid government issued photo identification, such as, but not limited to, a driver’s license, other state-issued ID or passport. In addition, the student must sign, in the presence of the institutional official, the Identity and Statement of Educational Purpose. This form will be available in Financial & Registration Services.
If the student is unable to appear in person at Castleton University Financial & Registration Services the student must sign and complete the Identity and Statement of Educational Purpose in the presence of a Notary and present a copy of a valid government-issued photo identification, such as, but not limited to, a driver’s license, other state-issued ID or passport that is acknowledged in the notary statement. Please print the Identity and Statement of Educational Purpose form found on the appropriate year Important Forms Page.
In signing the Identity and Statement of Educational Purpose, you are certifying the following to be true:
I will use federal and/or state student financial aid only to pay the cost of attendance at Castleton University.
I am not in default on a federal student loan or I have made satisfactory arrangements to repay it.
I do not owe money back on a federal student grant or I have made satisfactory arrangements to repay it.
I will notify Castleton University if I default on a federal student loan.
I will not receive a Federal Pell Grant from more than one college for the same period of enrollment.
I understand that purposely providing false or misleading information to obtain student financial aid is a federal offense punishable by fines and imprisonment.
I understand that the U.S. Secretary of Education has the authority to verify information used to apply for federal student aid with the Internal Revenue Service and other federal agencies.
I understand that Castleton University has the authority to request documentation required to verify the accuracy of my financial aid application, FAFSA.
I understand that to remain eligible to receive financial aid I must maintain Satisfactory Academic Progress toward the completion of my academic program.
I understand that withdrawing, dropping or not attending my classes may lead to the reduction or cancellation of financial aid and that it is my responsibility to consult with Financial & Registration Services prior to changing my course load.
I understand that Castleton University and the U.S. Department of Education will pursue collections efforts for cancelled or reduced aid that I received and to which I am not entitled.
